In Memoriam, 30 June 1971

The Soviet space mission Soyuz 11 was launched on June 7th, 1971. It carried three cosmonauts to the orbiting Salyut 1 space station; the only crew to successfully use it. After more than three productive weeks in space, the crew manned their Soyuz spacecraft to return to Earth. During the re-entry, a series of explosive bolts fired simultaneously instead of sequentially as had been intended. The resulting extra stress broke open a seal on a critical cabin ventilation valve, and cabin pressure was lost within a matter of seconds. The crew was found dead of asphyxiation after an otherwise normal re-entry. These three cosmonauts are the only men known to have died while actually in space.
