Question on writing a Dedication Oath

I am going to be starting my Path of the Druid and I was seeking help on what I should write in my Dedication Oath.  I am a solitary so this will be done by myself. I'm just having trouble with the specifics like what all to include, wording, and format.  There is a lot online about this but is more geared towards Druids in groves.  I also have questions about attire.  The resources on here are good for setting up my alter so I have that covered.
