Has there ever been a man that "killed" a god or deity?

Yes, but it wasn't permanent.

If a 'god' (or 'goddess') is no longer worshiped (forgotten over time, with no record kept, or destroyed) by 'man' (or woman) has he/she been killed?  A bit of a philosophical question, I suppose...

To answer this question you are going to have to do a bit more work.

You're going to have to start out by defining the parameters of what you mean by "god".

Then you are going to have to define what death means in terms of that definition of "god", keeping in mind that there are gods whose deaths are essential to their divinity (whether grain gods like Osiris or the Wiccan Lord, or apotheosised humans like Antinous, or other categories).

You will also have to deal with things like whether or not "forgotten" is equatable to "dead", especially given the existence of people who deal with neglected and forgotten gods, many of whose names have been lost.

Then you are going to have to determine what about a plausible death is sensically related to human action, given that the whole "And they took up the sword and stabbitied the god to death!" is a motif from fantasy novels, not religion, though it has mythological precursors.

Without a clear definition of parameters, there is no answer beyond things like "Jesus", who we know was killed by the Romans.  (And whose death is essential to his mythology, and thus is unlikely to satisfy as a "death of god" answer.)

... and, in a similar vein, if a dead god or goddess is worshiped again does the devotion bring them back to life?

Yes, actually, as several others have pointed out. Next question: Did he STAY dead? Looks not. Why? Well, while some look at those three days as some kind of pre-planned safari excursion, I see them more as the crack which remains once a shattered mirror has been laboriously put back together, piece by piece.
